Project Home Again

We offer two programs within Project Home Again (PHA) that provide permanent supportive housing for people who meet the eligibility criteria.

Project Home Again II

PHA II is permanent supportive housing for homeless people living with HIV/AIDS. These clients live as independently as possible in a permanent setting.

PHA II provides housing at various sites, as well as a housing facility with eight beds. It also provides an emergency bed for a person who is awaiting permanent placement.

Clients receive transitional living services, furnishings in the residential facility, case management, substance abuse and mental health counseling, and transportation.

Project Home Again IV

PHA IV is permanent supportive housing for homeless individuals and families living with a disability.

We are unable to respond to emergency needs. If you have a medical emergency, please call 911 immediately.

* The clinic at the Community Health and Wellness Center earned Federally Qualified Health Center Look-Alike (FQHC LAL) designation on 8/1/2019.
